Experience: Minimum of two years in a home health setting within
the last ten years License: Current RN Licensure in Client
Certifications: BLS required before start date Must-Have: - Two
years of experience in a home health setting within the last ten
years. - Experience conducting comprehensive assessments of
patients. - Experience working in a Certified Home Health Agency,
Licensed Home Care Service Agency, or Managed Long-Term Care.
Description: Required licensure includes current RN licensure in
Client. Required experience consists of two years within a home
health setting in the last ten years. Nurses must have recent
experience conducting comprehensive assessments of patients. All
nurse assessors must meet specific requirements. Preferred
qualifications include experience conducting assessments similar to
UAS. Preferred candidates will have experience caring for a
geriatric population and managing chronic diseases in a home
setting. Additional experience can include home care experience as
a paraprofessional or licensed practical nurse. Examples of
experiences that count toward preferred qualifications are provided
but do not satisfy required qualifications on their own. Candidates
must be under the impression they will be conducting hybrid tele
and in-field assessments. Candidates cannot participate in UAS
assessment assignments elsewhere. The ability to review health
status questions and make evaluations based on criteria for
enrollment is necessary. Candidates should work effectively with
technology in the workplace. A strong understanding of the
practices of professional nursing and relevant laws and regulations
is essential. Excellent organizational, interpersonal, written, and
verbal communication skills are required. Ability to communicate
and work effectively with multi-generational consumers is
important. Candidates must demonstrate sensitivity to the concerns
of others and perform comfortably in a fast-paced,
deadline-oriented work environment. The position involves executing
many complex tasks simultaneously and requires a strong ability to
work independently. Job responsibilities include performing initial
in-home evaluations of patients. Evaluations must consider health
status, strengths, care needs, and preferences. Nurses will review
consumer medical documentation relevant to the case. Data must be
entered into electronic forms and transmitted as required.
Documentation of concerns and other issues surfaced during
evaluations is necessary. Maintaining continuity of care and
reducing gaps in treatment plans is emphasized. Candidates will
discuss healthcare options, needed supports, and vendor options
with consumers. Communication with eligible consumers for
enrollment into managed care health plans will be initiated.
Enhancing communication and collaborative relationships with
interdisciplinary care team members is critical. Collecting quality
review data and supporting outcome measurements is part of the
role. Opportunities for health promotion and illness prevention
must be identified. A comprehensive working knowledge of community
resources and payer requirements for the target population is
essential. Meeting established standards and performing other
