Job Description:
Jersey Shore Post Acute, a leader in post-acute care and
rehabilitation, is seeking a compassionate and skilled Licensed
Practical Nurse (LPN) to join our dedicated team. In this role, you
will be integral to our mission of delivering personalized
healthcare plans and supporting our patients' recovery journeys.
You will work closely with a team of medical professionals to
provide top-notch nursing care, ensuring the comfort and well-being
of our patients. Administer medications and treatments as
prescribed by the physician Monitor patient health status and
respond to their needs Collaborate with a multidisciplinary team to
create and implement personalized care plans Document patient care
services by charting in patient and department records Educate
patients and their families on health maintenance and disease
prevention Provide emotional support to patients and their families
Strong clinical skills in a healthcare setting Excellent
communication and interpersonal skills Proficiency in medical
software and record keeping Ability to work collaboratively in a
team environment Medical Insurance Dental Insurance Vision
Insurance Valid LPN license in the state of practice CPR
certified
